fa بررسی و مقایسه اثربخشی برنامه توانمندسازی کارشناسان پرستاری و بهداشت در آموزش و کنترل دیابت نوع دو : کارآزمایی شاهد دار تصادفی شده لانه گزیده در یک اقدام پژوهی تخصصي Special پژوهشی-کمی Quantitative-Research <div><strong>مقدمه</strong> : این پژوهش با هدف بررسی و مقایسه اثربخشی برنامه توانمندسازی کارشناسان پرستاری و مراقبین سلامت در آموزش و کنترل دیابت نوع دو انجام شد.<br> <strong>روش کار:</strong> مطالعه‌ی حاضر از نوع کارآزمایی شاهددار تصادفی شده دو سوکور برروی 480 بیمار مبتلا به دیابت نوع 2تحت پوشش مراکز بهداشتی درمانی شهر اصفهان است که طی یک مطالعه اقدام‌پژوهی با طرح ارزشیابی لانه‌گزیده در سال 2018 انجام شده است. بیماران به صورت تصادفی به 4 گروه مساوی در دو گروه آزمون و کنترل با دو زیر گروه پرستاران و مراقبین سلامت تقسیم شدند.&nbsp; 2 گروه مداخله از پرستاران و مراقبین سلامت در 7 جلسه کارگاه توانمندسازی شرکت کرده و آموزش های خودمراقبتی بیماران خود را طی 4 جلسه اجرا کردند.<span dir="LTR"></span><br> &nbsp;اطلاعات دموگرافیک و هموگلوبین گلیکوزیله(<span dir="LTR">HbA1c</span>)&nbsp;بیماران در ابتدای مطالعه و سه ماه بعد از اتمام مداخله با روش‌های دقیق و قابل اعتماد جمع آوری شد. داده‌ها در نرم افزار <span dir="LTR">SPSS</span> نسخه 20 با استفاده از آزمون‌های مجذور کای، <span dir="LTR">t</span>مستقل، <span dir="LTR">t</span>زوجی، کولموگروف اسمیرنوف و <span dir="LTR">ANOVA</span> در سطح معنی دار 05/0 مورد تجزیه و تحلیل قرار گرفت.<br> &nbsp;<strong>یافته‌ها:</strong> چهار گروه بیماران مداخله و کنترل در دو زیر گروه پرستاران و ارائه‌دهندگان مراقبت ‌های بهداشتی در ابتدای مطالعه از نظر متغیرهای جمعیت شناختی و <span dir="LTR">&nbsp;HbA1c</span>تفاوت معنی داری نداشتند (409/0= <span dir="LTR">p</span>). سه ماه پس از مداخله، میانگین اختلاف سطح <span dir="LTR">HbA1c</span> بیماران در دو گروه مداخله (توانمندسازی پرستاران و ارائه دهندگان مراقبت های بهداشتی) در مقایسه با دو گروه کنترل معنی دار بود (003/0 = <span dir="LTR">p</span>) و بهترین تأثیر در گروه مداخله پرستاران مشاهده شد (003/0 = <span dir="LTR">p</span>).<br> <strong>نتیجه‌گیری:</strong> &nbsp;اثربخشی برنامه‌ی توانمندسازی&nbsp; و اثر بیشتر آموزش پرستاران در مقایسه با مراقبین سلامت بر بهبود شاخص گلیسمیک بیماران مبتلا به دیابت تایید شد. بنابراین اجرای برنامه توانمندسازی پرسنل بهداشتی و استفاده از نیروهای دوره دیده ‌ی پرستاری&nbsp; به دلیل نقش مهمی که در رسالت نظام سلامت در جهت بهبود شاخصهای کنترل دیابت دارند پیشنهاد می‌شود.</div> Introduction: The study aimed to compare the effectiveness of empowerment program for nurses and health care providers on glycosylated hemoglobin in middle-aged people with type 2 diabetes.<br> Methods: A double-blind randomized controlled trial was performed on 480 patients covered by health centers in Isfahan during an action research using nested evaluation design in 2018. Patients were divided into 4 equal groups of 120 cases in 4 groups of intervention and control in two divided groups of nurses and health-care providers. Intervention groups of nurses and health care provider participated in 7 sessions of empowerment workshop, then they thought their patients in 4 sessions about self-care issues based on their needs assessment. Patients&#39; data and glycosylated hemoglobin (HbA1c)at first and 3 months after intervention were collected by accurate and reliable methods. Data were analyzed in SPSS software version 20 with Kolmogorov-Smirnov, ANOVA, chi-square, independent t-test and paired t-test at a significant level of 0.05.<br> Results: Four groups of patients in the intervention and control subgroups of nurses and health care providers did not differ significantly in demographic variables and before the intervention(p=0.409). Three months after the intervention, mean difference of&nbsp; the patients&#39; &nbsp;HbA1c level between the two groups of empowerment (nurses and health care providers) were significant (p=0.003) compared with their control groups. Best effectiveness was seen in the nurses&#39; intervention group (p=0.003).<br> Conclusion: The effectiveness of the empowerment program on patients&#39; glycemic control was confirmed in both groups with higher effect of nurses&#39; education for A1c control in diabetic patients. Therefore, the implementation of this empowerment programs for health personnel and the use of trained nursing staff is recommended due to their important role in health system mission to improve diabetic control.<br> &nbsp; اقدام پژوهی, اثر بخشی, برنامه توانمندسازی, پرستاران, مراقبین سلامت, آموزش خودمراقبتی, دیابت نوع دو.
